Intermediate Care Facilities

Brookfield Centre

Brookfield provides a 30-bedded care unit for people who don't need to remain in hospital yet are not quite fit to return home; this could be for a variety of reasons. They also accept admissions from Urgent Care Response to prevent unnecessary hospital admission.

At Brookfield, intermediate care is provided to promote independence. The following service professionals are available to support the individual, and will help them to regain and retain independence:

  • Intermediate care support workers
  • Physiotherapist
  • Occupational therapists
  • Advanced Nurse Practitioners
  • Community psychiatric nurse
  • Social workers
  • GP

Needs are assessed and treatment is given in order for the person to be discharged home safely. The centre aim to review the person's needs within seven days of admission.

All referrals to the Brookfield Centre are made through the Contact Cares team at St Helens Council.
